try gangster #1 - british but disturbing or.. crap, there's one Japanese Gangster movie where dudes have to cut off their own pinky's instead of appologizing. Some dude disembowels himself rather than have his intentions questioned. I wish I could remember the name of it, but dude! It's crazy - I'll get back to you.

not japanese or italian but the "faces of death" series is pretty high on shock value. pretty disturbing stuff.
