I don't know if we really know what was real. In his mind, he went back and forth in time. In one scenerio, Jackie (Keira Knightly) was a waitress who was an alcoholic. Her mother had died by passing out and burning herself up. He was outside the diner on Christmas and she took him to her house. While there he found his dog tags he had given to her when she was younger. She told him that they couldn't be his, because he had died.

He visited Dr. Thomas Becker (Kris Kristofferson) and tried to make him talk about the treatment. He visited Dr. Beth Lorenson (Jennifer Jason Leigh) and told her she had written about helping her friends son come out of a coma like state by electric shock therapy.

He found out he had died from a head injury and he kept trying to find out what caused it.

He wrote a letter to Jackie's mom and had Dr. Lorenson drive him to her house. He basically told her that she was going to die and how, if she didn't stop drinking.

When he got back to the hospital, he fell and hit his head on the curb. He had them put him back in the jacket and he went forward in time to the diner where Jackie had been a waitress. She offered him a ride, but now she was driving a new VW and was a nurse at a hospital. While driving, her mom called her cell phone.

I recommend watching it because there is so much detail I can't put here. It is a great movie.

He died. They were trying to find out why they were lying to him about the blunt head trauma, but it turned out he slipped on ice and that WAS really how he died. He DID save the girl though! He stopped the mom from killing herself (burning to death) so the girl grew up to be a nurse? But that was his last "time travel" he died after that....at least, thats what I got from it.
