It's a combination of genetic predisposition and social forces. I've lived a long time, talked to many people (especially men) and quite frankly, I have yet to hear of one person who does not have at least one story that points to some family, school, or peer influence on their sexual orientation. And it's not just "how they are raised" either. Other social forces, especially during pre-adolescence and adolescence are influencing their orientation.
My guess is that bisexuality is actually quite normal for humans, especially men, but that particular orientation becomes "set" or "hardened" right around 12-15 years of age. Most bisexuals self-identify as such around their mid-twenties or so. It takes a bisexual much longer to accept his or her orientation than it does a homosexual person. And the early childhood "signs" that homosexual people often describe about themselves is not always present in the lives of bisexual people.
From my discussions with bi men, what seems to come up again and again are: (1) an over-socialization with one sex over the other in early childhood (either boys or girls) and (2) late childhood and adolescent issues over socializing with their own sex or the opposite sex. (Notice that I said socialization issues and not sexual issues.)
I've also noticed something with a few bi guys too: Around adolescence, there appears to be some issue that comes up regarding male body image -- some idealize the male form of other men and do not have a healthy self-image of themselves.
Some bi guys are very sociable around women; others are more sociable around their own gender. I would tend to say though that most bisexual men are in fact, largely, if not intensely "homosocial," preferring to be in the company of men (non-sexually) rather than women.
Most bisexuals do not act on the duality of their sexual orientation. And most keep their secret to themselves. A good number of supposedly straight fellows (very loyal to their wives or girlfriends) are actually bisexual.

Hard to say, I believe that it is an unatural state. I believe it Usually has to do with a parent that is either overbearing or an insignifiant nuturer in their lives. Eg. Boy's dad is hard as nails. Not a loving parent or mother is overbearing. Boy would tend to lean toward men for comfort. Vice versa for females. Bisexuality I think is more to do with human beings being very sexual creatures and enjoying experimenting. I have not heard of a bisexual Grandma or grandpa, so I would think most have laid their confusion to rest.

I think that most people are to some extent born bisexual and that they tend to gravitate to one preference or the other, whereas others stay in the middle.

Born. Just about everyone I know was raised to be heterosexual, as their parents were heterosexual. As a young boy I was told that in a few years I'd find girls to be really attractive (I wasn't told that I'd find other boys attractive as well). At some point, some of us just come to the realization that we are sexually attracted to both men and women. Others find themselves only attracted to those of one sex (their own or the opposite). This is because of something innate, not because of how we were raised.

Well both my parents and grandparents and aunts and uncles and brother are straight. If it was something I was raised with, my mom wouldn't be freaking out so hard about it.
