O from the mother
A from the father = A
O from the mother
B from the father = B
so.. nope

No this is not possible.
Type B could only come from a combination of a parent of B or AB blood.
A or O parents would only produce A or O typed kid

More blood tests on the parents I would suggest or a DNA test.

no it is not possible.
type A could either be AA, or AO
type O must be OO, therefore, the child could be A, or O, NOT B
