Because he has been in so many boxing matches. It is implied that that is how he lost his other eye, and why his face is so, um, messed-up. He has the typical 'cauliflower ears' and there is a lot of permanent swelling around the socket of his remaining eye. Apparently, he received that injury before he discovered spinach, so he could not regenerate from it.

He only squints with one eye, and that's the one that the smoke from his pipe affects. If he quit smoking he could see clearly with both eyes.
