Most of the time it's flavored tobacco, which gives you a kind of 'cigarette' high. It's not like pot or anything, but tastes good and it's really more of a social thing.
However, hashisih, keef and straight up pot are put in them sometimes, which of course gets your more 'high'. In American hookah bars you'll only see the flavored tobacco, and as far as I know in the middle east you gotta bring your own dope and keep it on the DL if you do.
