When a reporter prints or reports a story which leads to violence towards a person/individual, do you feel that reporter should be held accountable in criminal courts if the story turns out to be falsified, untrue, or out of context of the individuals words or actions?

Of course. Your name is credited with the article. Facts were a must when I was a reporter. Everything I put in there had to be based on facts ... if I wanted to inculcate my own input ... I needed to apply for the opinion column instead.

This isn't a difficult question to answer: if the report is false, then the reporter should be held accountable.

On the other hand, if the information is true and the report led to violence, that is a different matter, and should not be pinned against the reporter.
