It depends on the degree of invasion (how much the cancer has spread), and the type of cancer. Sometimes the doctor will do a "lumpectomy" followed by radiation therapy. A mastectomy is not always performed, but if there is a suspicion of the cancer having spread, it is better to lose a breast than to die of cancer.

It depends on how advanced the cancer is. Some people can just have a lumpectomy, others have a mastectomy and the most advanced is a radical mastectomy.
