The 'length' of the muscle makes a big difference. People with short muscles can 'buff up' to far greater extent than people with long muscles. Those with long muscles may work out regularly but never look "buff". But, they are better at things like cross country and marathons. People with short muscles can 'buff' up by doing very little. Likewise, if a person has short muscles and works out with weights, s/he will have much larger biceps than one with longer muscles.

It's genetic, and they are probably well fed- eat a lot of protein. They may be using their arms in everyday activities that you are unaware of.
