We charge a lot, and pay all our monthly bills, on a single credit card and pay the balance in full each month. In a year, we charge between $100,000 and $120,000. What are the best loyalty programs or points programs out there for someone like me us with this spending level?

I really like the CITI card program I have.
I get a no hassle cash back check in the mail, whenever I want.
I get 1% on all purchases and I get 5% at gas, supermarkets and drug stores. They tally up my points and when I reach 200 dollars or so, I just request a refund check and I get it in less than a week.

Wow, you're in a wonderful position to get some serious cash back.
Someone else asked a similar question here on answers.yahoo.com that I answered, so I'll parrot the same answer to your question:
Because of the different category percentages, tiers, limits, etc. of cash rebate cards, the best cash rebate card for you is a function of how much you spend in various categories each year. By categories, I mean, for example, gas, groceries, restaurants, utilities, etc.
Because there are so many different cash rebate cards with different rebate structures, the analysis required to find the best card is not trivial. And if you're interested in carrying more than one card and using each card for its strong suits to get even more cash back, then the analysis can get downright scary.
However, there is a free tool online that will do the number crunching for you and show you what you could expect to earn in cashback each year from the leading cash rebate cards. It'll calculate the best combinations of cards for you too. It's at http://www.creditcardtuneup.com/ .
If you happen to be doing your spending through a small business, there are some nice cash rebate business cards too. And they have high cashback percentages in categories that are different from the personal cards. That creditcardtuneup.com tool will do the analysis for business cards as well as personal cards.

good question!!! I also do that and I have Chase rewards. I have flown international a few times for free now. It has a 25 dollar annual fee but in 2 years I get a free international flight (value 1200 dollars to asia) so it is worth the 50 bucks in fee's.

Discover pays up to 1%, but I think all of them have limits of what they pay.
One percent per month at $10,000 per month would make you $1,200 per year, but I am sure that is over their limit. And if you are spending that much, $1,200 per year will not impress you, but it is free money.

If your charging that much, I would look into either american express, or discover card.
(we use discover, because we can still get rewards for not charging so much).
