I would say it increases.
1- Higher income populations are more demanding, tend to be more educated, and have higher expectations as to what options are available.
2- They have more to live for, and so are motivated to seek better mdical care.
3- They have more money, so medical practitioners are already more available, assuming a free-market economy, having gravitated to where the money is. Increased availability of services will stimulate demand.
