He was considered to old for the draft. He did volunteer for the marines, but was turned down. He then was asked to be in films in which he did very well in inspiring the country.

annie i reckon john wayne did serve his country by virtue of the fact he would have made some anti jap films to promote better feelings amongst those at home and those serving in the services.
Same as dean martin, sammy d jnr, frank sinatra and elvis no doubt

According to his biography, he had a punctured eardrum and a bad knee (from football). He did not evade or avoid the service. He was not physically qualified.
