A placebo, as far as I know, can do no harm, how canthere be an ethical problem?

The answer relies in the extent to which research participants gave "informed consent." Generally, during the "inform" part of the consent discussion, participants are told they may receive a placebo and if they do they may not receive the anticipated benefits of the actual test drug. On the other hand, they may receive some placebo effect. As long as the participant is informed of the possible alternatives and outcomes, the potential ethical issues are moot.
