The pain stimulates the nerve ending that relay it to brain. When brain recognizes this, the pain will be felt at that time suddenly, and shouting produced involuntarily depends upon the threshold of pain. If the pain is more shouting will be more. If it is less, no shouting at all.

well its jus the way u express urself...like in other situtations when ur sad u cry, when ur happy u smile...its js a reflex actionn to which one screams when in pain.

shouting actually releases endorphans to your body , making it less painful,endorphans are released when we smile as well
