Some terrible terrible luck (Steve Bartman) combined with some terrible terrible decisions and moves. They rely on some key players that are incredibly injury prone (Kerry Wood, Mark Prior) and have seen other key players go down (Aramis Ramirez, Derek Lee).
They've also suffered from bad managerial decisions, poor bullpen performances, disappointing rookies, over-reliance on bad players in key situations (Borowski, Dempster)... The list goes on.
That, of course, only covers their recent troubles. Historically they've suffered from similar things, but ultimately it's just an inability to put together a consistently performing team on the field.
