and my address, everything basically, what is the best thing that I can do? I do not have any idea of where I put the paperwork, but since I lost it I should assume the worst that it might be in someoone's hands now- how can I prevent identity theft or whatever might happen? Thanks...

Contact all 3 credit repositories (Trans Union, Equifax, Experian); ask them to put a fraud alert on your credit. They will place an alert/warning to contact you via telephone, etc. before extending credit. They all have toll free numbers. This does not need to be a writen request.

Report it to the credit card company ASAP. May be if you want to try to get a card in the future at the same place you could right now set up some sort of code word that only you and the credit card company know and then of coarse have everything confirmed before going ahead, the first and most important thing to do is contact the credit card company NOW !!!!
