Yes. For naturalized citizens, lying in the course of acquiring US citizenship. For those born with US citizenship, it cannot be "revoked" without the person's doing an "expatriating act."
Expatriation requires certain voluntary acts with intent to relinquish citizenship. In defending such an action, in which the government carries the burden of proof, I would present evidence tending to show lack of such intent. Note that the Department of State generally presumes that U.S. citizens intend to retain U.S. citizenship even when they are naturalized in a foreign country.

Yes, you can have your citizenship revoked. If you take an oath and pledge your royalty to other nation, join the military force for other nation, and few other that I can't recall, it is a ground for revocation.
Other than voluntary removal of citizenship, I don't know of a case where this law was called into effect.

If he were born here than unfortunately no. If he were naturalized it might be revoked and he would be returned to the country of his birth, but that would be highly doubtful. He should be displayed in a public square in stocks.
