I agree with the other two; you either need to know how far she drove, or how much she spent in all. Unless, however, they want an equation for how much she would spend if the mileage was a variable, in which case cost (C) = 12.50 + (0.25x) where "x" = the number of miles driven.

How many miles did she drive? If D = how many days she had the car, and M = the total number of miles she drove, then it's (12.5 x D) + (M x .25) =answer.

the miles she drove multiplied by 0.25 then add $12.50. Example:
she drove 45 miles
45*0.25=11.25
