The reason is you have not configured your speakers for Stereo sound,or you might be listening to a mono sound.

its plugged into the wrong plug, theres 3. 1 for left channel, 1 for right channel and 1 for both.

you must have 4.1 or 5.1 soundcard.

You could have a loose connection.

maybe one of the wires is cut, or you just have to plug then replug it

Mine do that, dunno why but to fix it i plug in my earphones then unplug them. Always works for me..
