how did people come to say "how in the hell?" or "what in the hell?"?

it just doesn't make sence to me

as they started saying........"what the fuc*".....where the fuc*,etc.....

there are certain elements in language that cant be defined...they just come naturally......

well people just referred hell to a bad thing, i guess. it just sorta came up

...i don't know but i have always wondered about this phrase:
"how about them apples"....how did that come to be?
