Just plain sickest and not necessarily scary? I'd say "Dead Alive". It was actually one of Peter Jackson's first flicks! Lots of camp, lots of gore... Who wouldn't like a movie featuring a martial artist minister that beats up zombies then proclaims, "I kick *** for God!"?
My favorite scene, though, would be where the hero is using a lawn-mower to tear up zombies, then it runs out of gas so he tries to run away but ends up running in place like in Scooby Doo since he is slipping on all the blood!

I believe the black and white Japanese horror movie Tom (from a few answers below) is referring to is called "Iron Man". That's a sick one too!

Haven't seen alot. I find horror movies don't have as much of an effect on me as most people. I'm not hanging at the bottom of my bed for six months if I see Saw. I'm just not a horror movie fan simply because I don't get scared. I'm more affected of suspence then blood and gore. Which most horror films are. I'm talking about movies like Blair Witch, The Ring, The Shining, Psycho (or any Alfred Hitchcock movie for that matter) heck even Jaws. You know what I mean. I can't really say because I don't get scared at those kind of movies.

The Hills have eyes. I know the movie was fiction and all but the storyline was more realistic than most of the horror films out there.

All sexploitation films are sick. "Hostel" is sick and perverted and just a sad atempt from Tarantino.
Then you have "Cannibal holocaust" which is one of the few I wish I had never seen at all, it polluted my soul.

Movies like that shouldn't exist. They just try to out-do eachother all the time, "who can make it even worse, who can make something even more sick"... Seems like it.

The Hills Have Eyes becuase it is soooo real when the father dies. Oh god i get goosebumps just thinking about it. Its crazy because that could actually happen.

One experimental black and white japanese film.. God knows what it's called, a friend showed me once when he was caned. This guy gets attacked by a mad cyborg girl.. he escapes but finds himself slowly turning into a machine.. at one point his penis turns into a gigantic drill and he kills his girlfriend.. it was seriously screwed up!!
