Oral sex is a major part of having sex. When you are kissing someone, you are basically having oral sex. Your mouth is on and in their body orifice! As far as cleanliness goes, most people are up on this especially if they know they are going to be having sex. Since I have no idea how old you are or your background, I would suggest that you talk with a parent , a peer, or doctor. It sounds like you need more in-depth answers than what you will get here.

All right I have read enough of this negative crap about oral sex and just can't help myself. well yah know, everyone is different and not everyone likes doing those things, but some of us find a lot of pleasure in them. as far as std's are concerned you should really know more about the people you choose for partners. heck asking isn't really that bad of an idea when in doubt. and for getting them in the mouth, well yah know, you can get them through other orifices too so it's like you still got the disease right?
as for me going down on someone and what's so great about it. well i guess you'd have to ask them that but i do enjoy pleasuring with my mouth and receiving pleasure orally as well. it's a whole different feeling and can be so intense.
