When my dearest love left me years ago, I know that I had a broken heart, because my chest actually ached. The emotional pain was hard enough to bear, but I was shocked to actually feel such an ache in my chest! I felt like there was a physical bruise, and for months afterward felt it bleeding a little every day. So yes, there really is a reason they call it a broken heart.
The good news is that we got back together, were married a couple of years later, and my heart feels healed - even if I still get weepy over sad movies or songs that remind me of that lost and lonely time. It takes a LONG time for a true broken heart to heal, for some.

yes. Your first one is the most painful and you actually will not be the same until it is mended. When the victim of a broken heart is ready to love again 100% with no bitterness, the broken heart is healed.

Yes absolutely. It's not physical - it's an emotional thing you feel especially when someone breaks up with you. Get the same feeling from losing a pet, etc.

Yes, both physically (as in heart disease etc) or emotional like the death of a loved one or saying goodbye to a best friend.
