Here's the origin of the phrase:

Yes, it means that things have turned out badly for someone, usually by their own doing. I once heard that the expression originates from many years ago when people used to use birds like geese to clean thier chimneys. The bird would be dropped down the stack and would naturally flap it's wings to fly out, thus cleaning the soot away in the process.The problem started if the bird got stuck there and the fire was lit by the person who dropped it down there - he cooked his goose!
