This is a disease of the kidney. Specifically a disease of the tubules of the kidney (tiny tubes that collect the urine) and the kidney stuff around the tubules.

It is most commonly thought to be an immune response - your body makes an antibody to fight something (can be a virus or an infecting bacterium, or maybe medications you are on), and it also happens to attack the kidneys.
