First and formostly stick all the pages together... leave to dry... then get a decent craft knife and cut out a square or oblong shape...going carefully through the layers.

i've done this. first off, get a nice big hardcover book. this is where you can get really creative or make a statement. maybe use a bible or the koran. personally, i went with a big book titled the history of france published in the 1950s.
start about a tenth of the way into the book and start cutting! i keep cash in mine, so i cut a rectangle. at this point it's sort of like carving out a pumpkin.
once the cutting is done, use duct tape or hockey tape to insulate the inside hole. time to glue. regular white glue (the kindergarten variety) works fine. you don't need to glue each page together, you just want to glue up the outside. let dry and reapply a couple of time to make a nice bond. make sure you don't glue the book closed.
that's about it.
i've kept my book-safe on the bookshelf for years.
