If it were me, i would not include any of that for a few reasons. 1. You dont want some psycho stalker talking to you just because he sees that you are tennis_girl@whatever.com. 2. Potential employers do search the web these days, and if they see osmething like bitchy_potsmoking_hotgirl@whatever.com youre not hired... lol. So my suggestion, first initial then middle intial, then last name. Or something of that sort. Keep it not very personal like your name.

Replace "15" with your actual age or the year of your birth or anything you can remember easily. The reason is to reduce the chance of spam. Spammers guess e-mail addresses that are too simple.
