Back-stabbing and a general overall personality of distrust and disloyalty.
Complete selfishness and no ability to understand the cares/needs/wants of others.

if you consist of a clear plastic bag and a long hose and you generally find yourself full of water and being inserted into a vagina ahh lets say once a month, you are probably a douche bag.
