Look at Article V of the constitution.

Short answer 2/3's of both houses of Congress + 3/4 of States must vote for it.

States vote for it by having it's legislators vote for it.

Well Congress has to pass an amendment to the constitution by a 2/3 vote. Or if 2/3 of the 50 states pass it as a law, then it can be added to the constitution.

It has to be put before the states to OK. Look what happened to the ERA
