How do you figure your belt size?

I know you are supposed to add or subtract an inch or two from your waist size, i just don't remember how much and which way.

I tend to subtract an inch, otherwise, if I get a belt that is the same size as my waist, I end up being on the last available notch, and not the one near the end. (In other words, I need to tighten it more, but there's no more holes)
