Wow. You are asking the question that brings fear to all of our hearts!!! The answer is: you don't. If you regularly do business with the same pizza place and see the same deliverers over and over again (and you tip well), you will probably be okay. But how do you know a disgruntled employee at any restaurant isn't sabotaging your food. You don't. Just be courteous to everyone (especially in the food serving industry) and hope they remember you with respect and good hygiene!!!

There is really no way of finding that out until you eat it and then sometimes you can't tell anyway. I worked for a pizzza place long ago and one of the chefs got mad at a patron and put anchovies juice all over the pizza. The patrons never said anything ....maybe they liked it I don't know. I know cooks in restaurants are infamous about spitting in meals if you send something back or have too many substitutions.

Look at it. If you see bubbles, or something that is obviously semen, don't eat it. And call the pizza place and talk to the manager, you can have a driver arrested for that, it's a felony if he has a disease.
If he peed on it, bon appetit, you'll never know.

If it has extra toppings that you didn't order or it's stuck to the lid of the box there is a good chance he / she did something to the pizza.

Umm, if there's white stuff on it that's not cheese?
The real answer is that you don't. You never know what you're eating, no matter where you get it. Anybody who's worked in a restaurant has probably seen some revolting stuff.
