Any animal large enough to catch it. Someone mentioned sharks, River otters are a tasty snack for mountain lions, wolves and, especially bears.
You might want to mention man as a potential enemy of the otter. Commercial fishermen, especially have demonstrated their resentment of the otter's protected status in certain parts of world. The otter feeds on the same foods that fishermen try to bring in for market. May otters have been found, shot dead by what is suspected to be resentful fishermen.

Human Beings. Here in the UK, people used to hunt the otter, purely for fun, the same way today that the fox is hunted for entertainment, by 'real men' (and, sadly, women).
Some new sort of insecticide, introduced in the early 1960's, decimated their numbers greatly.
