Whatever is the satndard specialist doctor visit for your area. It's around $160 where I live.

If you have insurance, it depend on your co-pay. Ususally $20 or 20%

If you have insurance it will be covered under that. You just have to pay a co-pay then. Mine is $30 per visit. If you don't have insurance I think where I live (Wisconsin) it's around $150 for the first visit and then around $70 for follow up appts. Just look up the dermatologists number in the phone book to the hospital you go to and schedule an appt. You usually don't need a referral.
