Naomi refers to Nelson Mandela as her grandfather as you can read from this article in The Observer from this link...
http://observer.guardian.co.uk/magazine/story/0,11913,1678568,00.html

Naomi: 'I've got my dad Chris Blackwell. My dad Quincy [Jones]. I've got Flavio [Briatore]. And I have Mr Mandela, my grandfather.'

Nelson Mandela also considers her an "unofficial grandchild" for all the charity work she has helped him with (The Mandela Children's Fund), but who started this unofficial family bond is not clear. In South Africa family relations are considered very important and that may be the reason for its use by Naomi. Nelson Mandela is after all royalty.
