No. It is recommended that to file for SSI benefits that you call SSA toll free at 1-800-772-1213 to make an appointment to file for SSI benefits either by phone or at your local Social Security office.

People who are deaf or hard of hearing may call the toll-free TTY number, 1-800-325-0778, between 7 a.m. and 7 p.m. on Monday through Friday. Please have your Social Security number handy when you call.

I do believe as with most SSI or SSA documents even if you can print them you still have to take them into an office. Its a security thing.

If your able, go to local SSI Office, let them fill'em out thats what they get paid for.

Atleast thats what I did.
