You need to buy a screen printer...go to your local art supply store...they will probably be able to help you with all the supplies you need

You've got two options
1. Go to a site that will print anything you want on a plain t-shirt like http://www.shirtcity.co.uk
OR
2. There are kits available that give you everything you need, including software and a t-shirt, or you can assemble your own materials. Whichever route you go, here's what you'll need for the typical iron-on style of transfer:
Software
Artwork
Transfer paper
Printer
Iron (or heat transfer press)
Hard surface (if using an iron)
Pillowcase or other fabric (if using an iron)
T-shirt or other item to receive transfer.

If its a light coloured t-shirt you can use your printer and print on to iron-on transfer paper. If there is writing on your image you need to print it back to front so it comes out the right way when you iron it on to the fabric, so you need a program to flip the image. Paint.net might do it.
If its a dark coloured t-shirt then go to a printing place and supply them with your image on cd.
Or you can upload your image to somewhere like Cafe Press or a number of photographic places such as Snappy Snaps, and they'll post back the t-shirt.

Buy an inkjet and transfer paper to iron it on (remember to flip the image or it'll come out back to front). Easier, and more professional (especialy if you want light colours on dark shirts - impossible with inkjet) go to a screen printer. It'll only cost about a tenner - well does here in Brighton anyway.

Lots of photograph studios will do this for you for very little money. Where I live, if you take a digital image in they will add any words etc. you want for about 16 Euro. They can also work from a photograph print but the quality is lower (also the price, about 8 Euro.)

get that special iron-paper that you put in your normal printer - print out whatever image you want from your comp & iron it onto your t-shirt.
