The TOEFL is the Test of English as a Foreign Language. It only tests your English ability (reading, writing, listening and now speaking; well, and your test-taking ability) and most universities in the US require it (some will allow equivalents) unless you have previously studied in an English-speaking country. Your score is only valid two years so don't do it sooner than you have to.

The GRE is required for many graduate programmes, the GMAT for many others. In the computer field, there is a chance that you can choose which of the two you want to take.

The GRE tests reasoning, reading ability, vocabulary, and writing ability in English, and math, while the GMAT tests reading, reasoning, grammar and writing in English and math. However, the writing is not nearly as important on the GMAT as it is on the GRE; on the other hand, the math is much harder on the GMAT. If math is your best subject and English is not your first language, I strongly recommend taking the GMAT if you have the choice.

The GRE also offers (separately) subject tests, which may be required for your programme.

For the GMAT and I'm pretty sure also for the GRE, scores are valid 5 years, so you may want to take that first.

Score requirements vary widely, so for that it's best to check with the individual schools.

This is actually many questions.
However, the answer to basically all these questions is the same:
Look at the admissions websites of the universities and departments you want to get into. See what their requirements are. Then follow those requirements.
Many colleges have reports available on the web that include, along with other information about the school, information about the people who were admitted to the school last year. You can compare yourself to those data to see what your chances of getting into the school are.
