Did the test, passed and lost the certificate. Can't find the link for the replacement certificates.

Driver theory test. I passed it a few months ago, but have lost the certificate. I know you can get replacement ones but I can't find the address on the net

If you have lost or misplaced your driver theory test certificate, you can obtain a duplicate copy. Simply write to the Driver Theory Testing Service to request a duplicate certificate.

You cannot request a duplicate certificate over the telephone. Your request in writing must include your name, address and PPS Number along with an 8 euro fee so your record can be identified. This fee must be included as a cheque, postal order or bank draft made out to Driver Theory Testing Service. (Do not send cash).
