Hi,

I need to start a system for a medical office for the patient files. When one patient chart is removed, we need to place something in that spot to mark the place where it was on the shelf, & for the employee to sign who took the chart, the date, and where it currently is.

My question is does anybody know what that those place holders are called & where they can be purchased? Is there a better system for the patient files than that?

I don't know what they are officially called but we call them checkout cards.

We keep a card with blank lines for name, department, in/out time, etc in a plastic protecter inside the file. whenever a file is removed the next blank line is filled out and left in its place.

it basically looks like a big library card (if you are old enough to remember library cards..heh)

We don't purchase them. We created a template in Microsoft Word and just print them as needed...hence the need for the plastic protectors.

I don't think this is restricted to the medical field. In any office that keeps record, if someone removes a file, you need a placeholder to indicate what the file is, and that it has been taken, with an indication of by whom, when, and for how long.

Why not have a piece of card in a distinctive colour that is easy to see, with the relevant details recorded on it.
