In the American League, in 1938, Bobo Newsom of the St. Louis Browns won 20 games but posted an ERA of 5.07.

In the National League, in 1930, Ray Kremer of the Pittsburgh Pirates also won 20 games, posting a 5.02 ERA.

Those are the only two pitchers to win 20 games with an ERA over 5.00.
