It could be any number of reasons. He needs to see a doc. My boyf twisted his testicle once which was really painful. The doc just gave him some strong painkillers until it sorted itself out.

The best thing to do is see a doctor, theres no need for him to feel embarassed they've seen every part of the human body before and least then a dr can put his mind at rest.

If you kick them really hard or squeeze really hard this will make them painful.
However he may have an infection if he is sexually active.
Try get him to go to the doc.It may be embarrassing but better than explaining to future partners he can't have kids.....
