It requires an order from a court in order to legally change your name. This requires filing a petition in court, and then the local newspapers must publish your intent in the legal announcements for 4 consecutive weeks. A hearing is set and then the judge will determine your case. if your requests are reasonalbe, and if you don't encounter opposition, then the judhe will decide to let you or not.
If your really serious, go to the Clerk of your County's District Court and ask. Also, look within your State's law library. The state law library may have offical forms approved by the state supreme court commission for your use. The forms may even be online. Check out your state's offical website.

In order to change your name you have to petition the supreme court in your county. The first step is to go to the office of the supreme court and request a name change petition.
The petition itself is not complicated. Some people hire lawyers for upwards of $500 to help them complete it, but this isn't usually necessary. If you make any large glaring mistakes on the petition the clerk of the court will usually just point it out to you.
You take your completed petition and filing fee back to the clerk of the supreme court and they will give you your court date and time. The judge will review your petition and may have some questions for you. His concern will be that you are not changing your name for illegal purposes or to avoid prosecution of a crime.
After he approves your petition you take your court order with you to the Social Security Office to change your name on your Social Security card. This will involve showing them your court order, presenting your old card (if you don't have it, a birth certificate will do) and completeing their simple form. They'll mail you a new card in a week or two.
You then do the same thing at the DMV to change your driver's license and the title & registration on your car. Just bring your court order, old license and title. They'll mail you your new forms in like 2-3 weeks.
Then just contact your bank, credit card companies, and utlities. Some of them will change it right over the phone and others will require you to mail or fax them a copy of the court order.
If you are changing your name because of marriage or divorce the process is simple. Just have it stated on your marriage certificate or in your divorce decree that you wish to change your name. You then just take the marriage certificate to the DMV and Social Security office...no need for court. If you wait until after you are married or divorced, then you will have to go to court.
Hope this was helpful! Best of luck!

One's first impulse would be to see a lawyer. But to save money, you might try one of those on-line legal firms such as Legalzoom. I am in no way affiliated with them.
