I'm in Year 7. There's a girl in my class named Grace, and she is gay. I want to tell her that she disgusts me.
How should I tell her?

You shouldn't tell her. You don't have to approve of her lifestyle. Keep it to yourself.

If you are a believer in Jesus Christ, ask yourself, "What would Jesus do?" Also, remember that "we all have sinned and come short of the glory of God." Sin is not graded on a curve like some school tests. Sin is sin. James 2:10 says, "For whoever keeps the whole and yet stumbles at just one point is guilty of breaking all of it."
Many times when I have been tempted to point out someone else's fault, I remember this statement someone once said, "When I point a finger at someone else, there are three pointing back at me."

Don't tell her. Why hurt her feelings? That would be pointless. As you get older, you may see a lot more people that disgust you. So then, what? You plan on going up to all of them and letting them know, and insulting them, do you? Nah, forget it. Turn your attention somewhere else.

Are you disgusted because she's cute and you want to get in her pants, and her being gay means you have no chance?

Keep your opinion to yourself. Otherwise you will become known as a heartless scumbag and eventually no one will like you and you will be alone and friendless.
