That is strange what revision? I work on those every day. Rack leaks then the pump makes noise. Clunk turning right to left and going over bumps is caused by a bearing in the steering column. Either way if you have been nice enough to get your Altima worked on at a dealership even if it is out of warranty Nissan might "Good Will" the repair to keep you as one of it's valued customers

You failed to say what kind of noise. If it only makes a grinding sound while turning then it is the power steering pump. The rack itself would only make a popping sound or a bumping sound if it where bent. If it is worn out it will leave a leak around the ends. Anything to do with steering is dangerous. What you most likely have is a bad power steering pump. There are additives at your local parts store that can help. It will only get harder and harder to actually turn the wheel.

the main thing a rack and pinion are used for is to help steering. with out it it is really hard to turn. it will help build your arms if you don't get it fixed. in the long run it will cost more money to fix it though.
