Ok it was episode 3.22 All Hell Breaks Loose, they were fighting a demon named Shax, and about the middle of the episode another demon reversed time, because their inoccent died, well Phoebe was in the underworld w/ Cole and Shax attacked Piper and Prue. Well it doesnt show you but in the next episode Piper was fussing at Leo because he saved her and not Prue.

End of season three she was killed by a demon when Phoebe was in the underworld and they could not use the power of three. Leo was also in the underworld and could not save both her and Piper so he saved Piper. Cole and Phoebe escaped the source ans Season four began with Prue's funeral.
