You call and make an appointment for while you're not on your period. Please be clean (for the dr's sake). You might want to bring a pantiliner or maxi for after, b/c they use a lot of lube to do the exam, and the pap test can make you spot a little bit. If you have ?'s, write them down on a piece of paper, and bring a pen with you, so you won't forget them and so you can write down the answers. The dr will probably sit down with you in his/her office b4 the exam to get a medical history and to answer your questions.
Congrats for taking control of your reproductive health!

When women become at least 18 or sexually active (whichever comes first) they should see a gynecologist.

About 48 hours before hand, DO NOT HAVE SEX! It will interfere with the results of a pap smear and make it look like you have the very early stages of cervical cancer. I know this for a fact because it happened to me. I had sex the night before and two weeks later got a call from the dr. saying I had the early stages of cervical cancer. I went in every 6 months after that for a year and everything was normal. I found out that the sex is what made the test abnormal.

Yes and like the other ladies said...MAKE SURE YOU ARE CLEAN DOWN THERE!!! The dr. does not want to deal with nastiness.
You should also make a habit of going to the gynecologist every year for a check up. This will help make sure you have no stis or other health issues.

If you are sexual active you do need to start going for regular pap tests for your own health. I don't know the protocol for where you live. In Canada I just go to my family doctor for this regular stuff and would just get referred to a gyn if there was something wrong. There are also clinics that have these services where you can go. While you are there you can get information about safe sex and sti'd etc.

Other than that I am not sure what etiquette you mean. Yes, you might want to be clean- but they recommend not douching or anything because it can interfere with the pap results.
