Send an e-mail with specific information regarding the nature of your illness and when you will return. Make arrangements to pick up any assignments or meet to discuss the material you will be missing.

Send him an email saying:
Professor, I am sick and will not be able to make it to class.
If you like, you can add a sentence asking if he needs anything from you (like a doctor's note) or asking for any assignments.
When I taught, I didn't really care if sick students sent an email. I posted class notes and assignments on the class bulletin board so that they could catch up.
