I checked the oil on my 1986 Honda Accord and it seemed low, so I added a quart. When I started the car next time, it sounded like it had a muffler problem, which has gotten louder since. The car also seems to be using fuel poorly and kinda sputtering. What could be wrong here? Could this be coincidental and not the result of my adding the oil? Might I have put the oil in the wrong place???

Did you check the oil level after you put the Quart in? You may have over filled it and increased the oil pressure which could put oil in places it shouldn't go. Since it's an old car oil might be getting past the piston ring because of the increase in pressure mixing a little oil with gas. Causing the car to sputter and more than likely smoke too.

I can't think of anywhere that you could have put oil that would cause what you describe. I would say it is just coincidental.
