Welcome to the world of American medicine. Why do you think the cost of insurance is so high? If I go to a doctor and pay cash, they charge $40/50. If you use insurance, check your bill and you will see the same visit is charged to the insurance company at $120.

My daughter's dentist puts all this stuff with prices on the bill that he makes me sign, but the way the form is worded it says he is doing these things even though I have told him I don't want certain ones done. He could easily submit all the items for payment to insurance and I am betting he does exactly that with the AHCCCS (govt) patient billings where the person who got the services doesn't see the bill to double check.

if you did i on purpose with the knowledge of it being wrong, then yes. you need to tell the Dr that you will not file anything with the ins unless it is correct.
I was working for an Auto Dealership. Used autos. I was dealing with the finance and the banks. Well, i had to lie on those forms and eventually quit because if the bank found out i was lying i could have gone down with the place. It eventually closed, but i am not sure why.

I suggest covering all your bases and reporting the inconsistency to a supervisor. If the doctor gets caught, then you will be in trouble for knowingly mis-charging customers.

NOT ONLY IS IT UNETHICAL, IT IS ILLEGAL. IT'S CALLED FRAUD.

IF YOU LIKE THE DOCTOR, LET HIM KNOW THAT YOU THINK HE MADE A MISTAKE. IT WILL GIVE HIM AN 'OUT' AND LET HIM KNOW YOU ARE ONTO HIM.

IF YOUR JOB IS THREATENED, IT WOULD BE A GOOD IDEA TO ALREADY HAVE COPIES OF THE FILES BEFORE YOU HAVE CORRECTED THEM (KEEP IN YOUR SECRET FILE).

IF YOU DON'T HAVE 'EVIDENCE' OR IF YOU 'OVERLOOK' AND DO NOT CORRECT, WHEN HE GETS CAUGHT, HE WILL PIN IT ALL ON YOU.

DON'T GET MIXED UP IN THE GAME. COLLECT 'EVIDENCE' AND PUT IT IN A SECRET FILE (FOR YOUR PROTECTION) AND LET IN KNOW YOU ARE ONTO HIM. HE MAY JUST STOP ON HIS OWN, BUT IF HE TRIES TO FIRE YOU, YOU'LL HAVE EVIDENCE OF HIS MISCONDUCT.

I think you need to report this situation to a Fraud Squad.

If you are aware of it and don't say anything..yes, you will be held responsible.

Get proof and send it to the media or a lawyer

Turn him in to the authorities. If you don't, you will be held liable as an accessory.
