I think this is a lovely gesture. Buy a blank card, write how grateful you are for the experience of working with him/her. How you appreciate the things he/she taught you, list a few, and that you will always be grateful that he /she was one of the important people to help you grow and gain in confidence and self-esteem.

I had to do the same thing earlier this year (I was offered a big promotion at a different location within the company). I asked my boss to have coffee with me before work. I thanked him for mentoring me and for treating me so well as an employee, and apologized for putting him in the position of having to find a replacement. I wished him the best in his career, and gave him a small corporate gift (he had a thing for pens, so I got him a nice one). Do something thoughtful that shows you respect him (or her) and will miss working with him. Most managers understand that losing employees is part of business, and if you were good enough to be offered your own restaurant, he probably had an idea it was coming anyway.

Hi there, get him a nice thank you card that has the thoughts that you want to say in it. Then you can sign it and leave it on his desk on your last day. Make sure you have a letter of recommendation from your boss on company letterhead before you leave. Good luck!

I simple thank you for the opportunity should be okay. If you have not already done so and are able to do so, offer to help fill the position before you leave to sitting in on interviews and explaining you position in full to prespects. His angery may be over the thought for trying to replace an employee as good as you!
